Three to four months. That is the answer you will get everywhere and it is accurate as an average.
It is also useless if you are one of the people getting eight or ten weeks and wondering what you are doing wrong.
What determines duration
Dose. The biggest factor by a wide margin. Undertreated muscle recovers faster. If you are getting a light dose to keep the cost down, you are also buying a shorter result. That is a legitimate trade and it should be a conscious one.
Muscle strength. Strong muscles metabolize the effect faster. This is why men typically need more units and often still get shorter duration.
Metabolic rate. People with high metabolisms tend to break it down faster. Endurance athletes are the classic example. If you run 40 miles a week, expect the short end.
Treatment area. Crow's feet often wear off before the forehead. The masseter often lasts longer than the upper face.
Treatment history. Duration frequently improves after your first two or three rounds, as the muscle weakens with repeated treatment and starts requiring less.
Antibody formation. Uncommon but real. Some people develop resistance over years of treatment. If yours has progressively shortened over several years, mention it.
What does not shorten it, despite the internet
Facial massage. Saunas. Exercise the next day. Sleeping on your face after the first night.
The one exception is the first four hours, where we ask you to avoid lying flat, rubbing the area, or strenuous exercise. After that, live your life.
The zinc question
There is a small body of research suggesting zinc supplementation may extend duration. It is not conclusive and the studies are limited.
If you want to try it, talk to your doctor about dosing. It is cheap and low risk and it is also not a reliable fix for a dosing problem.
The pattern to watch for
If your Botox consistently wears off at eight to ten weeks, the most likely explanation is dose, not metabolism.
Have that conversation with your injector directly. Ask what your unit count was and whether increasing it is reasonable for your muscle strength.
A good injector will discuss it openly. If yours dismisses it, that tells you something.
The maintenance argument
People who treat consistently every three to four months generally report that the effect holds better over time and that they need less product eventually.
People who let it fully wear off and come back twice a year tend to start from scratch each time, since the muscle has fully recovered its strength.
If budget is the constraint, treating one area consistently beats treating everything twice a year.
When to book your next appointment
Book at the point where movement is returning but before the lines are back. For most people that is around week ten to twelve.
Waiting until you look exactly like you did before means the muscle has fully recovered.
Do not book earlier than ten weeks. Overlapping treatments too closely increases the risk of resistance developing and does not improve results.
